In order to prepare for your appointment and to ensure all of your health concerns can be addressed, please bring the following:

• New Patient Registration Forms
• Your Insurance Card(s)
• A government-issued photo ID
• Any pertinent past medical records
• A list of current medications you take (prescription and non-prescription)
• Contact information of any specialists or other physicians treating you

For your convenience, you can complete the above new patient registration forms prior to your appointment. After you complete the forms, you can:

• Print the completed forms, sign all pages where indicated, and bring with you to your appointment.
  OR
• Save and email the completed forms to us at: friscoprimarycare@gmail.com. We will obtain your signature on these forms when you arrive to your appointment.
